Manually.
Depending on your layout, you also could put your character in one layer and set it to "immune to camera", which will move everything else around, but this might give a very mechanic result.

Lots of different ways to do this, however, they have one thing in common: you have the original layer, plus one or more FX layers over and under it. Usually I start with just duplicating the original, then layer blur it. Just play around a bit with blur and opacity settings and blend modes.

Agreed. In real life lots of sentences require no lip movement at all. The anime way with just closed - half open - scream works for a lot of scenes. No, as far as I understand Rhubarb uses the Preston Blair system, which means you need to name your layers exactly like that, with upper/lowercase. Layer order isn't used here.
