How do I get full opacity for my custom brush when using "Tint brush using stroke color"?
It always comes out transparent.

I'd rather like someone to tell me I'm wrong (and also tell me how to get tint to work at 100% opacity) but ...

do you have any parts of your brush that are NOT 100% black at 100% opacity? If so, I think that explains it because (IIRC) white and transparent are more or less equivalent and result in the "tint" being applied at less than 100% opacity - by exactly how much depends on the whiteness or transparency of the non-fully-black areas.

I think that any coloured part of the brush which isn't 100% black is interpreted as being transparent. I'd see this as a feature because if I need that brush's shape in 100% opacity and uniform colour I could copy it and make it an ordinary brrush.

Just confirmed that it needs to be all black to be opaque.

Well in that case, that passage in the manual:

"Tint brush using stroke color: For brushes that are colored (not all black) this option allows the brush to take on the selected stroke color instead of the color displayed in the brush selection menu."

Is actually saying the opposite of the reality.

I've just logged a couple of suggestions.
1) get the manual's wording sorted out (J338)
2) add an option to brushes to use opacity / whiteness differently (J339)
-- opacity as is (to give transparency in the stroke);
-- white (and all shades of grey) at 100% opacity in the brush to give 100% opacity in the stroke but blended with with the palette colour
